Fort Worth ISD students have access to new school-based clinic

Fort Worth Independent School District students have access to a new John Peter Smith school-based health center. The new clinic is located at the Chapel Hill Academy, 4640 Sycamore School in Fort Worth.

The new health center will serve students and their siblings currently attending Fort Worth ISD schools, Crowley ISD schools and Lena Pope Home charter schools.

Typical services include:

  • Treatment for sore throats and colds
  • Immunizations, assessment and treatment of minor injuries
  • School physicals
  • General physicals such as checkups for Texas Health Steps

JPS Health Center Hours are Monday through Friday, 8:00 a.m. to 12:00 noon and 1:00 p.m. to 5:00 p.m. The clinic is closed most school holidays.

The per visit co-pay for students without insurance is $5.00. For students with insurance or other coverage such as CHIP, Medicaid, or JPS Connection, applicable co-pays and PCP requirements apply.
